Community Health Assessment/Community Health Improvement Plan

The Community Health Assessment (CHA) is conducted by the Knox County Health Department every five years to identify key health issues through data collection and analysis. The primary goal is to capture the current health status of our community, informing strategies to enhance health in Knox County. This assessment is utilized by various sectors including local nonprofit organizations, hospitals and community partners.

The Community Health Improvement Plan (CHIP) is a vital document that guides efforts to enhance our community’s health and operates on a three-year cycle. The current CHIP focuses on three key priorities: Adverse Childhood Experiences (ACEs), Cancer and Diabetes. To address these priorities, the health council has established subcommittees, also known as working groups, which collaborate with community partners and members to promote health and well-being.
