Community Health Improvement Plan
The Community Health Improvement Plan is a roadmap to improvement based on the priority health issues selected by the Community Health Council. Released on a three-year cycle, the Community Health Improvement Plan is the defining work of the Community Health Council and serves as a guiding document for community stakeholders and policymakers, alike.
The Community Health Council identified three priority health issues for Knox County in the 2024-2027 Community Health Improvement Plan. Diabetes, adverse childhood experiences (ACEs), and cancer were selected as areas of focus for the County, with respective target goals for the three-year implementation phase.
